Your First Visit
To simplify your first visit, please fill out our New Patient Form* online prior to your appointment or arrive 10 minutes early to fill it out in office.
*IMPORTANT: Any form submitted by email must be encrypted, to ensure it is secure. By submitting an unencrypted email form, you are consenting to an unsecured email. Alternatively, please print off the form, complete it, and bring it to our office.
To start, your dentist will review your dental and medical history, then perform a comprehensive dental exam. This exam will include a thorough examination of your teeth, gums, jaw and bite, as well as an oral cancer screening and any x-rays that may be required.
Based on your examination and history, a long-term dental treatment plan will be created for you. Your dentist will also dedicate time to answering any questions you may have about your dental health and treatment options.

- How do I cancel or reschedule an appointment?
To cancel or reschedule an appointment, please call us. Please notify us of any change of plans as soon as you can, so we can offer the space in our schedule to another patient.
- Do you offer appointment reminders?
In the days before your appointment, you will receive automated phone, text or email appointment reminders from us. If you need to cancel or reschedule an appointment, please call us as soon as possible to let us know.
- Do you provide services in any languages other than English?
Yes, our office is able to provide dental services in several languages including French, Cantonese, Spanish, Portuguese, Hindi, Punjabi,Tamil, and Vietnamese.
Please advise us of any languages preferences you have when booking your appointment.
- Does your clinic have on-site parking?
Yes, free on-site parking is available for all our patients. You can also reach us easily via public transit.
Insurance & Financing

- Direct Billing to Insurance
For your convenience, we offer direct billing for most insurance plans. We can submit your insurance claim electronically, resulting in faster reimbursement, without the requirement for paper forms and signatures.
Our administrative team can help you understand your insurance plan better and can also send estimates to your insurance company prior to beginning any major treatments.
Please bring your insurance plan information to your appointment.
- Payment Methods
Any co-pays or amounts not covered by insurance are your responsibility, and must be paid at the time of your appointment.
We accept payment by cash, cheque, debit or most major credit cards.
